To schedule an appointment

Call our office at 281-494-1314 to schedule your appointment. If you cannot keep your scheduled appointment time, please contact as soon as possible so that we may use that time for another patient who needs care. We sincerely make an effort to adhere to our appointment schedule and appreciate your patience if we are running behind due to an emergency.

Fees and payments

Please contact our office between the hours of 8:00am - 3:00pm, Monday through Friday, to request all prescriptions and authorizations for refills. Our staff has access during this time to our full records. Requests for prescription refills made during other times will be filled the next working day. Prescriptions requested after 3:30pm on Friday will not be filled until the following Monday.

Payment is requested at the time our services are rendered.  We accept cash, check, MasterCard, Visa and Discover for your convenience. If we participate with your insurance company, we will file your insurance for you. You will still be responsible for any amounts above and beyond what is included in your insurance coverage.  Our capable staff is available to assist you with the handling of your insurance claims.

Prescriptions and refills

If you are a new patient, we ask that you arrive early for your appointment so that you can complete the registration process.


Please bring the following information with you:

• Driver's license, State ID or passport

• Referral (if required by your insurer)

• Insurance card or Medicare care      

• Copies of medical records, x-rays, MRIs and CT scans from prior doctor visits

• A list of current medications and any known allergies

• If the patient does not speak English, please bring an interpreter
